Health Transitions Medicaid Outreach

Data shows that parents who are either on Medicaid or Medicaid eligible (low/no income) are more at risk of child maltreatment. Therefore, providing outreach services to this population is strategic and effective in diminishing child abuse. The Medicaid Outreach program helps parents identify parental and family needs, learn about and access Medicaid benefits that support and increase child wellness, improve school readiness, and connect to other community-based resources. CAPS staff provide public health insurance enrollment assistance to families not on a public health plan but who are eligible.
