We are proud to offer numerous services to homeless, runaway and at risk youth from as young as 12, to as old as 21.

With multiple services offered this flow chart was made to help make finding the right program fast and easy.

Rapid-Rehousing for Youth

Goal

Family Stabilization and the prevention of youth homelessness for 25 families annually.

Connection to longer term services leading to housing stabilization for 350 youth annually.

Family Reunification whenever safe and in the best interest of the youth or alternative housing for 40-50 youth annually.

Exit to safe and stable housing for 20-25 youth annually.

Transition in place; youth takes over lease & lives independently for 6-8 families annually.

Transition in place; youth takes over lease & lives independently for 13 youth annually.

Eligibility

  • Youth ages 12 through 17
  • Experiencing conflict at home
  • At risk of running away or separating from the family home without immediate intervention

 

  • Youth ages 12 through 21
  • Runaway or been kicked out or asked to leave
  • Couch surfers without family support & the means to live independently
  • Literally homeless
  • Youth ages 12 through 17
  • Runaway
  • Been kicked out or asked to leave by caretaker or head of household
  • Entire family has been displaced and youth cannot remain with family
  • Youth ages 16 through 21
  • Homeless without regular safe & stable housing including couch surfers
  • Youth asked to leave by adult caretaker or head of household
  • Pregnant or Parenting Youth
  • Ages 17 through 21
  • Homeless or without safe and stable housing for themselves and their child/ren

 

  • Youth ages 18 through 21
  • Homeless without regular safe & stable housing

 

Priority

  • Family is willing to engage in services to resolve conflict & stay together
  • Youth has been disconnected from systems of support for a lengthy period of time
  • No other immediate safe housing solution exists
  • Aged out of foster care
  • Family reunification failed or not possible
  • Youth who are homeless as a result of pregnancy or having dependent children
  • Referrals from Street Outreach & other homeless providers
  • High TAY-VI-SPDAT

Services

  • Assessment of family strengths and needs
  • Service plan to meet the family’s unique needs
  • Individual therapy for identified family members
  • Family therapy to resolve family conflict, rebuild relationships and improve communication
  • Home-based services available up to 90-days
  • Immediate needs assessment
  • Basic care needs: clothing, food, hygiene products, transportation
  • Case management to assess, plan, and support longer term housing and basic care needs
